Hi!, I would like to make a request for a copy of Certificate.

GOOD DAY, NAVY!

I'm Wendel Moore L. Cosep from Lapu-lapu City, Cebu. A Passer Candidate Soldier of Philippine Army and was a Marine Transportation Student in University of Cebu Lapu-lapu and Mandaue last 2014.

I would like to ask a copy of my Certificate  Completion and Serial Number in ROTC last March 2012, Conducted by the 504ᵀᴴ RESERVE OFFICERS TRAINING CORPS UNIT. For the reason that, it's required in the process for employment on Enlisted Personnel in the Philippine Army.

Dear Wendel Moore,

Greetings!

We regret to inform you that this is not a valid FOI Request. You may proceed to or call your ROTC unit/Reserve Center where you finished your ROTC program. Present your authenticated Certificate of Completion to the Admin and Personnel Officer of the unit to retrieve your records and/or the Special/General Orders indicating your Name and AFP reservist serial number.

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to paoafp@gmail.com. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.

If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011).
