Hi! I would like to request a Statement of Service of late enlisted personnel PO2 Orlando P. Rivera,Phil Navy 571010.dated January08,1971-December 18,1995.
It is a requirement for the Posthumous Retirement claims and benefits and for the amendment of his optional retirement order issued to him by Phil Navy,AFP.
The veteran died last December 17,1995.(shooting incident)
The claim accrued in 1996
