Hi Good day I’m Jomery Mortel Alonsagay and I would like to request urgently my serial number in ROTC certificate, because it is a requirement for my graduation I took my ROTC at Colegio De La Purisima, Roxas City Capiz 2013-2015. I don’t have now my certificate because it was flooded and i transfered here in Romblon State University, thank you for your kind and consideration God Bless!

Response to your request

We regret to inform you that this is not a valid FOI Request. You may proceed to or call your school/ROTC unit/Reserve Center where you finished your ROTC program. Present your authenticated Certificate of Completion to the Admin and Personnel Officer of the unit to retrieve your records and/or the Special/General Orders indicating your Name and AFP reservist serial number.

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to paoafp@gmail.com. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.

If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011).
