Hi, I would like to request a document of implementing guidelines and procedure of recharacterization of discharge order. Process to make an appeal to ammend the discharge order from dishonorable to honorable discharge.

1.	A fully accomplished AFP FOI Request Form (Annex F) with reasonable description of the information being asked
2.	Two valid government-issued IDs
3.	Clearances from the NBI, PNP, and Barangay
