I am currently working on my thesis concerning mental health program for the military 

may I request for the list of mental health programs and its description of DOH and other government agencies. 
Looking forward to your prompt response to my request.

1. A fully accomplished AFP FOI Request Form (Annex F) with reasonable description of the information being asked
2. Two valid government-issued IDs
3. Clearances from the NBI, PNP, and Barangay
4. An endorsement letter signed by your research adviser that also details the scope and purpose of your study.
