Good day DOH team, I am humbly requesting for a copy or sample of the above request. Our military treatment facility is currently crafting the hospital manual and we hope to source out references for this. I hope for your kind assistance on this. thank you and God bless

Greetings!

We regret to inform you that, per Section 5 of the AFP FOI Manual, in order to process your request, we will need you to submit the following:
1.	A fully accomplished AFP FOI Request Form (Annex F) with reasonable description of the information being asked
2.	Two valid government-issued IDs
3.	Clearances from the NBI, PNP, and Barangay
