I would like to request an incident report regarding the fire that happened at Salvation Army, Vincent Drive, Gun-ob Lapu-Lapu City last 13th of July 2019 as required for my CignalTV account disconnection.

Kindly indicate my sister's name Mavil Abayon as resident affected by the fire incident happened on the said location.
