February 8, 2022 Dear Eric, Greetings! Thank you for your request dated Feb 05, 2022 under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. You asked for Fire Report for incident that occurred on 12/11/2021. Unfortunately, we cannot provide you with the Fire Incident Certification Form as it is a Frontline Service of the BFP and therefore, is an exception in the Freedom of Information documents that can be requested. Please go to the Manila Fire District at Rizal Avenue corner Quiricada St., Sta. Cruz, Manila. You can also call them at Landline Numbers (02) 309-9648 / 43 / 45. If you are unhappy with this response to your FOI request, you can ask us to carry out an internal review of the response by writing to DIRECTOR LOUIE S PURACAN, Chief, BFP, at ofcbfp@gmail.com. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.
