Good day!

I am requesting a copy of my father's BIR Form 2316 for the fiscal years 2020 and 2021. This is because it serves as a requirement for college scholarships for the school year 2023–2024. We have tried reaching their HR department, but they said they could not provide any more copies as they already gave their last one to my father on November 17, 2022. Unfortunately, my father lost it on his way home. I am hoping for your utmost consideration.

Greetings!

Thank you for your request dated Nov 22, 2022 under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

You asked for Request for BIR Form 2316 and/or Income Tax Return.

Response to your request

We highly  suggest that you ADVISE your FATHER to submit a REQUEST HIMSELF and attach a Scanned Copies of Two (2) valid Government I.D.s containing your Father's Birthdate, Photograph and Signature.

Please be informed also, pursuant to Section 21.3 (e) of the Revised People's Freedom of Information Manual, Chapter VII-Standard Procedure for eFOI Requests, if the requesting party is a minor, his or her guardian or any competent supervising adult shall comply with the requirements of the representative. 

The authorization shall expressly state the extent of the authority of the representative to make the request, provide clarification, receive the requested information, and to bind the principal for all representations and/or undertaking made by the representative in connection with the request for information.
