Hi! I would like to request for a TIN Verification Slip for opening a bank account and also for verification purposes.

Thank you for your request dated Aug 15, 2022 under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

You asked for Request for TIN Verification Slip.

Response to your request

In reply, you may proceed with our Revenue District Office (RDO) No. 109-Tacurong, Sultan Kudarat on your request for verification. Please bring two (2) valid government-issued I.Ds containing the photograph and signature as proof of your identity. If the request is made through representative, a written authorization and/or special power of attorney of the requesting party (principal) and at least two (2) valid government IDs containing the photograph and signature of the representative shall be presented.

Please be reminded that TIN is confidential information that is bounded by the Data Privacy Act of 2012.
