Good day! I am Jan Alexeis C. Lacuata, an ENT Doctor. Would like to kindly ask if I can be furnished a copy of my annual ITR for 2023? This is for purposes of applying for Philhealth accreditation. Or if not, instructions on how to proceed with this? Thank you!

Dear Mr. Lacuata:

I refer to my letter of Jan 15, 2024 06:31:23 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for REQUEST FOR COPY OF ANNUAL ITR YEAR 2023.

This is in connection with your request for information, with reference number #BIR-415052149534, for the Request for a Copy of Income Tax Return.

After careful consideration, copies of Income Tax Return (ITR) can be requested from the Revenue District Office (RDO) where the same was filed and/or submitted or to the RDO where your Taxpayer Identification Number (TIN) is registered. You just have to submit a Signed Letter Request for the purpose. 

However, if you are referring to BIR Form No. 2316, the same shall have to be requested from your EMPLOYER since the same is the One Mandated by Law to Prepare and Provide to the Concerned Employee/s pursuant to Section 2.83.1 of Revenue Regulations (RR) No. 10-2008.

Also, we would like to clarify if you are really asking for your FIled Income Tax Return  (ITR) for Calendar Year (CY) 2023?, because it is yet to be file on or before April 15, 2024.
