I hope this letter finds you well. I am writing to request a copy of my Income Tax Return (ITR) 1701 for the financial year 2022.
My name is QUEENIE MAE B. BAGANI, and my tax identification number is 461-656-705-000. I am in need of a copy of my ITR for my visa and credit card application.
I would be grateful if you could provide me with a copy of my ITR at your earliest convenience please.

I refer to my letter of May 29, 2023 04:53:39 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Income Tax Return Copy 1701 for Year 2022.
In order for us to work further on your request, we need more information from you regarding your query. In particular, it would be helpful to know the following details:
1) What is the Complete name indicated in the Income Tax Return (BIR FOrm
1701) you filed?
2) What is your Tax Identification Number as indicated in your filed return?
3) What Revenue District Office No. did you file your form 1701?
4) Did you file online (eBIR Form Facility) or over the counter?
Please be informed further that this office does not have copies of the “No Payment” returns filed either through eBIRForms or eFPS.

4. I filed it thru eBIR Form and paid the tax due using my gcash. Attached is the requested transaction history from Gcash. Tax paid is Php 5,000.

We are continuously verifying your payment in our system but so far we have not found any record in our database.
While our aim is to provide information whenever possible, in this instance this Office does not have [some of]* the information you have requested.
