February 17, 2023 Dear Ms. Corpuz: Greetings! Thank you for your request dated Nov 17, 2022 11:01:43 AM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your request You asked for BIR Form 2316. Response to your request We called your mobile number today, February 17, to validate your existing eFOI request and you informed us of the following: 1. You have resigned from the company and has an ongoing work dispute filed at the Department of Labor and Employment (DOLE); 2. No BIR Form 2316 was issued to you; In this regard, you may verify with the compliance section of RDO 44-Taguig/Pateros (as this was the address you have provided in your email) on whether or not the said company (Everise) has filed your Income Tax Return/BIR Form 2316 (in your behalf) together with Annex F of Revenue Regulations No. 11-2018. We relay to the Taxpayer Identification Number (TIN) which is not provided as the request for a copy of BIR Form 2316 shall be made/endorsed at the Head Office of the Company where registered.
