Dear DEVINE NICHOLE PEREGIL,

I refer to my letter of Aug 04, 2023 11:19:01 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for 2021 Income Tax Return and/or BIR Form 2316.

For consideration purpose, kindly provide the folloiwng:

1. Complete Name of your Employer for CY 2021;
2. TIN of your employer;
3. Registered Address of your Employer, we will be endorsing your request to the Revenue District Office (RDO) where your employer is registered. They will be providing you a copy of your BIR Form No. 2316 for CY 2021 if your employer has filed in your behalf.

You may also ask from your employer copy of the Duly Received “Annex F” received by the RDO.

For other concern, email sherry.ann.arcita@bir.gov.ph and indicate in the subject email ticket number #BIR-428138609793, to respond.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Oct 30, 2023 02:10:12 AM, your request will be closed.
