I would like to request your good office for a Certified True Copy of my ITR (BIR Form 2316) filed for the calendar year 2021.  The said copy will be used for Pag-IBIG Housing Loan.

Dear Mitcheal Amarante:

Greetings!

I refer to my letter of Jul 25, 2022 08:40:03 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Income Tax Return (ITR) and/or BIR Form 2316 for Taxable Year 2021.

If you need a Certified True Copy (CTC) of your ITR and/or BIR Form 2316, you may personally or through your authorized representative file your request to the Revenue District Office (RDO) having jurisdiction over the place of business of your employer, kindly bring the original return and a photocopy of your ITR and/or BIR Form 2316. You can also ask from your employer copy of the Duly received Annex F (the proof the employer came to Revenue District Office) and have the List & BIR Form 2316 of their employees who availed of substituted filing. Then bring a copy of 2316 of the requesting party for stamping of certified copy.

Any request for certification that may be requested by a taxpayer from the RDO where he is registered on matters relating to its registration shall be charged with a fee in an amount not exceeding one hundred pesos (P100.00), in addition to the documentary stamp tax imposed under Section 188 of the National Internal Revenue Code, as amended.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Oct 18, 2022 04:45:15 AM, your request will be closed.
