Thank you for your request dated Nov 13, 2023 under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

You asked for Request for Certified True Copies of the Audited Financial Statements of Jurassic Freight Services.

Response to your request

Pursuant to Executive Order No. 2 S. 2016, any request for information through eFreedom of Information (eFOI) portal (www.foi.gov.ph) shall attach scanned copies of  the following documents:

1.	Letter request by the taxpayer or its authorized 
        representative;
2.	The request shall reasonably describe the purpose of 
         the request for information;
3.	Original Board Resolution duly notarized, Corporate 
        Secretary Certificate, or other acceptable written 
        consent (Special Power of Attorney) showing the 
        authority of the representative to act in behalf of the 
        company.
4.	At least two (2) valid government I.D.s containing 
        photograph and signature of the taxpayer and 
        representative.

Your request will be evaluated by Revenue District Office (RDO) where this company is registered. Hence, other confidential matters falling under the exceptions to the Freedom of Information are prohibited to be released by the concerned RDO.
