Dear Ms. RAMOS,

I refer to my letter of Aug 07, 2023 07:14:47 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Income Tax Return and/or BIR Form 2316.

In reply, kindly provide the following details/information:

1. Name of your Employers for Calendar Year 2020 to 2022;
2. TIN of your Employers;
3. Registered Address of your Employer.

The concerned will be endorsed to the Revenue District Office (RDO) where your employers are registered. They will provide a copy of your requested documents if your employer has filed in your behalf.

The RDO might be requiring from you a copy Annex F duly received by the RDO and was submitted by your employer to them.

Email the requested information at sherry.ann.arcita@bir.gov.ph and indicate in the email subject ticket number #BIR-445147237371, to respond.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Oct 31, 2023 08:47:24 AM, your request will be closed.
