I would like to request copies of my Income Tax Returns for calendar years 2019 and 2020. My name is Eric Christopher Ouzts and my TIN is 453-490-236. I was employed by TelePhilippines (Teleperformance Philippines) during this time period.
I was instructed to attach copies of two (2) valid IDs but there is no attachment option. Can you please provide an email address where I can send the copies of my IDs?
