Good day.
I want to request, if possible, a stamped copy of the Annual Income Tax Return that I filed for Fiscal Year 2022. Likewise, I request a stamped copy for each Quarterly ITR I have filed so far (2022 Quarter 2 & 3, 2023 Quarter 1-3).
I make this request to have legal documents that will support my claim of self-employment, which I wish to declare in my tourist visa application to Australia.
I hope the request could be granted; or if not, that I receive clear instructions on how to obtain the documents I have mentioned.

Dear Regibelle Lyn Palo,
I refer to your letter dated Nov 05, 2023 12:44:43 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Stamped Copy of Annual ITR for FY 2022 & all filed Quarterly ITRs.
In order for us to proceed with your request, we need to have a copy of your quarterly filed ITRs. It would be helpful to know if you paid your returns by bank because, as per our systems in the Bureau, there are no records found.
Here are the requirements on how to obtain Certified True Copy of Returns:
1. Written Request indicating the Return being requested and the specific purpose of the request.
2. Photocopy of Two (2) government-issued IDs w ith 3 specimen signatures.
3. Certification Fee (counter 7/eFPS) and Documentary Stamp (counter 7).
If transacting through a representative, additional requirements apply:
• Special Power of Attorney (SPA) indicating the Return being requested, the name of the representative, and the specific purpose of the request.
• Photocopy of a Valid ID with 3 specimen signatures of the Representative.

A personnel in my RDO (#21A) contacted me as well, informing me of the requirements. However, I forgot to write down her name.

As already stated, I've received a call 2 weeks ago from my registered RDO (21A), where a personnel informed me of the requirements, and that I must appear in person to have the certified copies. My only problem now is that I forgot to write down the name of the female personnel who called me.
As for the copies of my quarterly filed ITRs, I have already informed the female personnel at RDO 21A that I filed mine electronically, receiving the confirmation receipt via email. The RDO personnel and I have agreed that I will print those emailed confirmations and present them to her in person along with the uncertified copies of my quarterly ITRS.

In particular, I had requested specific information for you to clarify upon in order for us to proceed with the request.
As we have not received a reply within the 60-day working period since the letter requesting for clarification was sent out, we regret to inform you that this request will be closed as of Feb 02, 2024.
You may wish to submit a new request describing the specific information you are looking for, and we will be able to consider it under the guidelines as stated in Executive Order No. 2.
Your right to request a review
If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to or visiting our central office. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter.
