I would like to request for a copy of my BIR 1905

Dear Mr. Lao:

I refer to my letter of Dec 18, 2023 04:29:11 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for BIR 1905 Form Request.

In reply, please be informed that BIR Form No. 1905 is accomplished in two (2) copies the 2nd copy – old RDO’s file copy and the 3rd copy – taxpayer’s file copy; 
You may file your request directly to the Revenue District Office (RDO) where you submit the aforesaid form.

It is also helpful to provide your specific date of visit to the RDO concerned so they may release the duplicate copy of the form in less time.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Mar 12, 2024 01:01:27 AM, your request will be closed.

We hope to hear from you at the soonest possible time.

Thank you for your reply. I was informed that my RDO code is 124. Not sure where it is since I am here in Davao. Is there any way I can get a copy of my 1905 electronically?
