Greetings!

Thank you for your request dated Aug 01, 2022 06:52:20 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

You request for a Certified True Copy (CTC) of Income Tax Return (ITR).

Per our phone conversation this afternoon of August 2, 2022,  relative to your CTC request, may we request you or your authorized representative to submit the following requirements to RDO 110 - General Santos City located at BIR District Office Building, Pendatun Avenue, cor. Laurel & m. Roxas Street General Santos City the following:
a. Duplicate/xerox copy of filed ITR (BIR Form 1701) for validation of the rightful owner of the documents requested;
b. payment for certification and documentary stamp; 
    (P200 - certification fee if with attachments and 2 pcs. doc. stamp)
    (P100 - certification fee without attachment and 1 pc. doc. stamp)
c. Special Power of Attorney for authorized representative with 2 valid IDs;
d. 2 government IDs of taxpayer;
