Hi! I would like to request for a copy of my ITR for the year 2021 which I have filed last January 14, 2022 based on the email notification i have received that day. I was not able to secure a copy of it and I need to for employment purposes. Thank you for your consideration and I am looking forward for a positive response.

1) What is the Complete name indicated in the Income Tax Return you filed?
2) What kind of Income Tax Return are you requesting from us? Is it BIR Form
1701? or BIR Form 1701A?
2) What is your Tax Identification Number as indicated in your filed return?
3) What Revenue District Office No. did you file your Income Tax Return Form?
4) Did you file online or over the counter?
