Hi i would like to request a copy of my 2316 form, my form was lost due to fire and ive ask mayvprevious employer and was advsied that they wouldnt be able to issue a new one since they gave me the original copy of it

I refer to my letter of Jun 24, 2021, 06:14:46 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Lost 2316 form.

In order for us to work further on your request, kindly clarify if what you are requesting is BIR form 2316 for 01/29/2021 - 02/26/2021, as if you are correctly requesting the same, please be informed that this form has not yet been filed to our office. You may just request from your employer a certified true copy of this form.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Sept. 17, 2021, 01:43:23 PM, your request will be closed.
