1. Are you a Local Employee during the year/s 2014-2016?
2. Or Self Employed Individual (With Business, Practicing One's Profession or Under Job Order)?
3. What Income Tax Return (ITR) are you referring to, is it BIR Form 2316 for Employees or BIR Forms 1701 & 1701A for Self-Employed Individuals?
4. Name of your Employer/s last 2014-2016?
5. Employer's Business Address; and
6. Employer's Business TIN (if applicable).

In order for us to work further on your request, we need more information from you regarding your query. You have indicated in your request that you require a copy of your ITR for taxable periods 2014-2016. Based on the records that we have, you are a registered taxpayer of RDO 41-Mandaluyong. Your request was instead forwarded to RDO 53A- Las Pinas due to the information that you were employed by our registered taxpayer. For verification and clarification, kindly provide additional information on the following:
1. Did you have a registered business between the periods 2014-2016? Or were you an individual earning on purely compensation income from the period of 2014-2016?
2. If employed, kindly provide the name of your employer and registered address of your employer.
3. Withstanding your answer to the first and second query, did you file an ITR for the cited periods?

Dear Ms. Armie Lyn Simeon,
Thank you for your response regarding my request. Here are the additional information you need:
1. I was an individual earning on purely compensation income for the period of 2014-2016. I do not have any registered business during that period.
2. I was employed under Communities Bulacan Inc. This is the same company that processed my monthly salaries. Communities Bulacan Inc. is a company under Vista Land & Lifescapes Inc. I am not sure which one processed my income tax.
3. It was the HR who filed for our income tax in the company.
I hope I answered all the information needed to proceed my request. Thank you.

Based on the data that our Office have, your supposed employer has no record of Annual Information Return on Income Taxes Withheld on Compensation for the periods 2014-2016.
Please be informed that you can request your employer a copy of your BIR Form 2316 pursuant to Section 2.83 of Revenue Regulations No. 02-98, as amended.
