Hi! May I please request for copies of my Certificate of Compensation Payment/Tax Withheld (BIR Form No. 2316) from year 2010-2013?
Unfortunately I lost my copies due to calamities and I am in need of these documents for my current employment requirements.
If it helps, I believe my RDO Code is 049.
At the very least, can I please get a formal statement from your office stating that I have paid tax from year 2010-2013, and indicating my employer's name?

I refer to my letter of Sep 28, 2021 10:21:01 AM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Certificate of Compensation Payment/Tax Withheld (BIR Form No. 2316).
Response to your request
While our aim is to provide information whenever possible, in this instance, this Office does not have the information/documents you are requesting.
Please be informed of the below procedure in ITR Filing from 2013 onwards:
1. The employers provide for the BIR Form 2316 to their employees after the year ends; and
2. Subsequently, all employees are required to file their ITR returns to the RDOs where they are registered, with tax due or zero/no payment, hence, we cannot provide a copy of BIR 2316
The mandatory submission of Employees’ BIR Form 2316 in lieu of the Income Tax Return (ITR), “who are qualified for substituted filing” by the employers started the year 2013, reference-based in RR No. 11-2013 and further amended by RR 2-2015.
However, in order for us to work further on your request, it would be helpful to know where you exactly filed your ITR and 2316 from 2010-2013 so we can properly endorse your request to the concerned RDO.
During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Dec. 21, 2021, 10:36:03 AM, your request will be closed.

Thank you for your quick reply and for the information. I believe my 2010-203 ITR or Form 2316 was filed in North Makati (RDO 049).
Also if it helps, my employer’s name at that time is BAICH INCORPORATED.

As per Integrated Tax System your TIN : 287-808-524-000 is registered with RDO 24 Valenzuela under EO 98 instead of Local Employee. You may wish to contact your previous employee to facilitate your request for BIR form 2316.
