I am Jerry Sarmiento, and I am writing in through this website to request a copy of my BIR 2316 form. I would like to have a copy from my previous employer named  CCER Megafood ventures from February 20,2019-August 2021. 
I badly needed it because it will serve as one of my requirements for my employment.
I would really appreciate if you will send  me a copy through email and I am only given until this 28th of December to comply.
I am hoping a positive feedback regarding this request.
