I refer to my letter of May 03, 2023 07:10:58 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Income Tax return and/or BIR Form 2316.

In order for us to work further on your request, we need more information from you regarding your query. kindly provide the following information:

1. Name of Employer/s for Calendar Year 2021 and 2022;
2. TIN of Employer/s;
3. Registered Address of your Employer/s;
4. Your Birthday;
5. Your Address;
6. Your TIN; and
7. Your contact number (mobile no.) in case we need to call you for further info.

Kindly email the requested information at sherry.ann.arcita@bir.gov.ph and indicate in the subject email ticket number #BIR-505662489537, to respond.

As an added information, BIR Form 2316 for Calendar Year 2014 cannot be provided to you since the record of the RDO on those period is unavailable since we follow the strictly the four (4) year retention period of the document. It would be practical to request for a copy to your former employer as this should have been issued to you as a matter of right (as well as your 2021 and 2022 BIR Form 2316).

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Jul 27, 2023 04:36:10 AM, your request will be closed.
