I would like to request the following BIR forms that I have submitted to BIR RDO 93A located in Zamboanga City.

BIR form 2551M covering the period of January to December 2017
BIR form 1701 covering the period of FY 2018 to 2019
BIR form 0605 covering the period of FY 2017

I will be using these forms as an attachment to close all my open cases at BIR RDO 93A.

Good day!

Per our discussion, they will tag my request as complied and require me to submit a request letter addressed to the District Revenue Officer for them to act on my request formally. I forwarded my request last August 14, 2023, through the email address they provided (see attached file). On August 25, 2023, I followed up on my request through the same email address but I have not received any reply. For the second time, on September 4, 2023, I sent another email to follow up on my request. I hope that this message will bring attention to the concern. Thank you!
