Hi I would like to request a copy of my BIR Form 2316 from 2021-2022. I will be needing these as required document in our process to purchase a house. I ak currently living in the US and unfortunately I dont have the copy of my BIr Form 2316. I hope you can help with this matter. Thank you!

I refer to my letter of Aug 16, 2023 12:09:01 AM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for BIR FORM 2316.

In reply, please provide the following details:

1. Name of your Employers for the Calendar Year 2021 to 2022;
2. Taxpayer Identification Number (TIN) of your Employer; and
3. Registered Business Address of your Employer.
