Hi. I would like to request for a copy of my form 2316. Here are my details:

Employment Dates: Feb 01, 2017 to Jun 30, 2018
Agency Address: Antipolo City
Requested form 2316: for the years 2017 and 2018

I tried requesting for copies of my form 2316 from the said office but they said they have already disposed all their files. They have not given me copies of my form 2316 when I was employed and when I was not renewed from my position.
