Hi! I would like to request a copy of my 2316 form for pre employment requirements i recently lost my copy of it

You may ask your employer re: BIR Form No. 2316 for this year(Certificate of Compensation Payment/Tax Withheld) as this should be issued by them. As mandated by law, every employer shall issue BIR Form No. 2316 to every employee before the close of such calendar year, if employment is terminated, on the day on which the last payment of compensation is made.
