Dear Sir/Madam,

Hope you are doing well.

I am writing in through this website to request and hopefully receive a copy of my BIR 2316 form. This is for the purpose of submitting one of the employment requirement for my new employer. Regrettably, my previous employer is not able to provide me with my BIR 2316. I worked from July 2022 to December 2023, would be very much appreciated if you can send me a copy via email as my employment is on the line — I have to submit this on February 2, 2024.

In reply, you did not provided us any Identification Card for your query. Please provide us a valid government for Identification purposes and we can further assist you with your concern. You can attach any of the following: SSS, GSIS, National ID, Philhealth and TIN ID Card.
