Issued to: melody L. Pagsanjan
Issued on: march 11, 2020 as payment for Funeral services amounting to 35,000.00

The original receipt can no longer be found and I need a CTC of it for death claims to be filed to SSS.

While our aim is to provide information whenever possible, in this instance this Office does not have the information you have requested. Also, I have noticed that the Official Receipt was issued to Ms. Melody Pagsanjan. You may just advise Ms. Pagsanjan to submit a request herself or just request from Villamor Funeral Services as they are the main source of said receipts. The said funeral services should be the one to issue the CTC not the BIR.
