Hi i would like to request form 2316 or the income tax return showing my address. This is for the coins.ph to verify my account. I hope you can help me. Thank you. Or any form that you can send that has my address will be also fine if the form requested is unavailable.

Dear Ms. Mahilaga,

I refer to my letter of Feb 09, 2022 01:57:04 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Income tax return form 2316.

In order for us to properly endorse and work further on your request,  please be specific what documents you want us to look into f it is ITR or 2316 as we have different policies and manner of filing said documents.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before May 05, 2022 12:25:44 AM, your request will be closed.
