October 21, 2021 Dear Mr. Zaragoza, Greetings! Thank you for your request dated Oct 20, 2021 under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your request You asked for Bir Form 1701Q Income Tax Return and Form 0619E. Response on your request While our aim is to provide information whenever possible, in this instance we would need for you to resubmit your request as we are still missing from you some vital information. Kindly provide us additional contact details and attach two valid IDs with your birthdate and picture, for identification purposes. Furthermore, please be informed that the ITRs that we have in our files are those returns that were filed and paid over-the-counter through our various Accredited Agent Banks; however, for no-payment returns that were filed through our eBIRforms and EFPS system, we do not have the hard copies of these returns on file. Also, If this has been filed online, there must be a Tax Return Confirmation Receipt for each return filed, thus, you need to check these documents from your end. For your guidance, please refer to the Revised People's Freedom of Information Manual for the Bureau of Internal Revenue, which is available for download at the Bureau's Official Website or www.bir.gov.ph. Thank you. Respectfully, Ana Marie Du FOI Officer
