I would like to request for a copy of my form 2316 for year 2021.
I left my previous last February 2022 and got a new job March 2022.
I have been requesting this since March but my previous employer still hasn't issued my Form 2316 unti today as I send this request ( 05/28/2022)

I will truly appreciate your assistance on this matter

I refer to my letter of May 28, 2022 08:19:56 AM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for 2021 FORM 2316.

While our aim is to provide information whenever possible, in this instance we would need for you to resubmit your request as we are still missing from you some vital information. Please provide us your Employer's Name for 2021, Employer's Business Address and Employer's Tin and email it at sherry.ann.arcita@bir.gov.ph. As the additional information requested from you contains sensitive information. Indicate in your email subject with ticket number #BIR-542953592659, to respond.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Aug 23, 2022 04:58:14 AM, your request will be closed.
