Thank you for your request dated Apr 14, 2023 04:19:36 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

You asked for re: change of civil status.

We called your mobile number today, April 18, to validate your existing eFOI request, however, it was unsuccessful.

Please be informed that you may send your duly accomplished BIR Form 1905 and at least two scanned copies of your government issued I.D.s and your marriage contract to the Revenue District Office (RDO) where your TIN Number is registered.

You can now retrieve your Taxpayer Identification Number (TIN) and Revenue District Office (RDO) Code through Revie’s TIN Inquiry Module. Revie can be accessed on the BIR website at https://www.bir.gov.ph.
