Hi Ma'am/Sir, 

I would like to request for a copy of my BIR Form 2316 form for the year ended 2019 because I can't recover now my copy of it. I just want to use the said document as a proof of shortpayments during those period.

Dear Mr. Cortez:

I refer to my letter of May 02, 2023 04:54:25 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for a copy of BIR Form 2316.

In order for us to work further on your request, we need more information from you regarding your query. Kindly provide us the following information:

1. Complete Name of your Employer (during the Calendar Year 2019);
2. TIN of your Employer;
3. Registered Address of your Employer;
4. Your Birthday;
5. Your Address; 
6. Your TIN; and
7. Your contact number (mobile no.) in case we need to call you for further info.
