You asked for Certificate of Compensation Payments/Tax  Withheld.

Response to your request
While our aim is to provide information whenever possible, in this instance this Office does not have the information you have requested. However, you may request a copy of BIR Form 2316 from your previous employer as it is mandated by law that every employer shall issue BIR Form No. 2316 to every employee before the close of such calendar year, or if employment is terminated, on the day on which the last payment of compensation is made. 

On the other hand, you also need to attach two valid IDs with your birthdate and picture and your contact number, for identity verification` purposes.

Mam wala na daw copy ang employer namin na isubmit na daw po sa bir noong february nabasa kasi po yong sa amin kaya naitapon po namin na tanngap po ba ang bir kung sakali pupunta kami don para mag request nito
