Hi,

I hope this letter finds you well. I am writing to request a copy of my Income Tax Return (ITR) for the financial year 2023.

My name is Kathleen Joy S. Capitulo, and my tax identification number is 359-751-373-000. I am in need of a copy of my ITR for my personal record.

I would be grateful if you could provide me with a copy of my ITR at the earliest convenience. 

Thank you for your assistance in this matter. I look forward to your prompt response.

Dear Ms. Capitulo:

I refer to my letter of Jan 16, 2024 06:48:43 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Request for a Copy of Income Tax Return (ITR).

In reply, we would like to make the following clarifications:

1. What kind of Income Tax Return (ITR) are you requesting from us? Is it BIR Form No. 1701 or BIR Form No. 1701A?
2. What is the Taxpayer Identification Number indicated in your filed ITR?
3. When did you file your ITR?
4. Did you file it via eBIR Form Facility or Manually at the Revenue District Office?
5. Are you requesting for the CY 2023? or CY 2022? 

Please be informed that the CY 2023 ITR is yet to be file on or before April 15, 2024.

Kindly email the requested information at sherry.ann.arcita@bir.gov.h and indicate in the subject email ticket number #BIR-582939969670, to respond.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Apr 11, 2024 06:15:11 AM, your request will be closed.
