I refer to my letter of Jul 04, 2023 08:24:09 AM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Income Tax Return and/or BIR Form No. 2316.

Per our phone conversation, kindly provide the following:

1. Complete Name of your Employers for the CY 2020 to 2022;
2. Employers TIN;
3. Work/Employment Tenure in the Company; and
4. Employers Registered Address.

Email the requested information at sherry.ann.arcita@bir.gov.ph and indicate in the subject ticket number #BIR-594643088970, to respond.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Sep 26, 2023 01:27:05 AM, your request will be closed.
