Hi, i would like to request a copy of my 2316 for the year 2020. I Have reason to believe that my former employer was not correctly remitting or may not have remitted at all my witholding taxes for that year as other Gov Rem (SSS, HDMF, PHIC) have lacking months in them.

I asked my employer for a copy of my 2316 to which up until today, they have not provided.

I refer to my letter of Jun 12, 2021 09:43:07 AM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for 2316.
