Greetings!

Thank you for your request dated Nov 09, 2022 03:59:24 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

Your Request

You asked for your TIN Number.

Response to Request

Upon verification from our database we have found out that you do not have a Taxpayer Identification Number (TIN) yet. You may apply for a TIN Number at the Revenue District Office (RDO) having jurisdiction over your residence address and submit the duly accomplished BIR Form 1904 (downloadable at the BIR Website www.bir.gov.ph). 

Attached any government-issued ID (e.g. PhilID, Birth Certificate, passport, driver’s license, Community Tax Certificate) that shows the name, address, and birthdate of the applicant, in case the ID has no address, any proof of residence. (1 photocopy).
