Good day Mr. John Lui, This is regarding your inquiry on (eFOI) about the Latest Filing of your Annual Income Tax Return(AITR). Upon verification, the latest filing of your AITR is for the Taxable Year 2021. To get a copy of your AITR, you may request for a Certified True Copy to this district office.

 

Below are the necessary requirements for Certified True Copy:

 

1.       Request Letter

2.       Certification Fee – P100.00

3.       Documentary Stamp – P30.00

4.       Photocopy of Valid ID w/ 3 specimen Signature

5.       Duly accomplished Freedom of Information Form

6.       If Transacting with Representative;

a.       Special Power of Attorney

b.       Photocopy of Valid ID of the Representative w/ 3 specimen Signature

 

Note: You can pay for certification fee and Documentary stamp at our cashier counter.
