How may we request for ITRs or BIR form 2316 with certified true copy stamps?

Dear Maria Rosabel Ang:

I refer to my letter of Jun 15, 2022 01:41:51 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for BIR 2316 Certified True Copies.

In relation to your request for a CERTIFIED TRUE COPY of BIR FORM 2316 for taxable year 2021, we would like to ask for the Name, Business Address and TIN of your Employer so we can properly endorse your request to the Revenue District Office (RDO) where your employer is registered and where they submit their reports.

An original copy of BIR FORM 2316 given to you by your employer should be presented to the district office in order for them to verify if it is the same as the one on their records.  A Certification Fee of P100 is required per copy.

Kindly email the needed information/details at sherry.ann.arcita@bir.gov.ph indicating in the subject email ticket number #BIR-613083201383, to respond.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Sep 07, 2022 11:24:03 PM, your request will be closed.

Your employer “Circles Life Technology Philippines Inc” with Tin Number 010-430-132-000. As per our verification, your employer did not submit a list and copy of their employees who availed of Substituted filing together with the form 2316. We regret to inform you that no records were filed with us.
