Dear Ms. [Redacted][Name] Limon:

I refer to my letter of Apr 21, 2022 11:23:26 AM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Income Tax Return.

In order for us to work further on your request, and to properly endorse the same to our Revenue District Office (RDO), we need the following information.

Kindly clarify if what you are requesting and referring is your BIR Form No. 2316 for employees or BIR Forms No. 1701 and 1701A for Self-Employed Individual, as we have different policies, manner and venue of filing on these documents. 

For BIR Form 2316, please indicate the Name of your Employer or Employers for Year/s 2012 - 2014, their Business Address and TIN (if applicable);

For BIR Form 1701 and 1701A, please provide your Trade Name, RDO where registered and Tax Return Confirmation Receipt (if your ITR were filed online).

Please email the above requested information at vanessa.badeo@bir.gov.ph as there are sensitive information being requested from the email. Indicate the Tracking Number for easy reference.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Jul 18, 2022 02:18:08 AM, your request will be closed.
