October 17, 2022 Dear Mr. Espinili: Greetings! Thank you for your request dated Oct 15, 2022 under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your request You asked for Request for TIN Verification Slip. We would like to advise you to visit the nearest Revenue District Office (RDO) on your request. Please bring two (2) valid government-issued I.D.s containing the photograph and signature as proof of your identity for validation purpose. If the request is made through representative, a Special Power of Attorney of the requesting party (principal) and at least two (2) valid government IDs containing the photograph and signature of the representative and the principal shall be presented.
