Dear Ms. Rose May Ramos:

I refer to my letter of Apr 26, 2022 06:28:53 AM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Income Tax Return.

Kindly clarify if what you are requesting and referring is your BIR Form 2316 for Employees or BIR Forms 1701 and 1701A for Self-Employed Individual, as we have different policies, manner and venue of filing for these documents. 

For BIR Form 2316, please indicate the Name of your Employer or Employers for Taxable Year/s 2020 and 2021, their Business Address and TIn (if applicable);

For BIR Forms 1701 and 1701A, please provide your Trade Name/Business Name, RDO where registered and Tax Return Confirmation Receipt (if your ITR were filed online).

If you filed using the eBIRForms Package, you can print a  file copy of the filed return and email confirmation for your tax return filing in eBIRForms for future reference.  If you were not able to print one, you may retrieve your filed return by selecting the form in the eBIRForms profile and then click the “PRINT” button below the form.

Please email the above requested information at sherry.ann.arcita@bir.gov.ph as it contains sensitive information, indicate the Tracking Number #BIR-640271544721, for easy reference.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Jul 19, 2022 06:52:49 AM, your request will be closed.
