Greetings!
I am Jem Elizshel G. Alcantara, Requesting an ITR form for the year 2019 - 2020. I was employed with Big Outsource and wanting to update and file form ITR 1701 because I am now Self Employed. My Tin #: 750-223-114-000. I have been late to file my Form 1701 due to Covid reasons.

Dear Ms. Alcantara,
I refer to my letter of Aug. 12, 2021, 09:36:42 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for ITR BIR Form 2316.
In order for us to work further on your request, we need more information from you regarding your query. Kindly provide us your employer's TIN, business name & address and also your TIN & email it @ ana.marie.du@bir.gov.ph
However, if you have been employed and your income tax has been correctly withheld, has only one employer, and has been qualified for substituted filing for the taxable year 2019, the Certificate of Withholding filed by your employer, duly stamped "Received" by the Bureau, shall be tantamount to the filing of the income tax return. You may request Form 2316 from your employer as it is mandated by law that every employer shall issue BIR Form No. 2316 to every employee before the close of such calendar year, if employment is terminated, on the day on which the last payment of compensation is made.
Furthermore, you may still file your ITR for 2020, however, you may already incur penalties for late filing.
During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Nov. 05, 2021, 02:20:15 AM, your request will be closed.
