Hi,

I hope this letter finds you well. I am writing to request a copy of my Income Tax Return (ITR) for the financial year 2022.

My name is AURELIA RABANG MANANQUIL, and my tax identification number is 408723253. I am in need of a copy of my ITR for my visa application.

I would be grateful if you could provide me with a copy of my ITR at the earliest convenience. 

Thank you for your assistance in this matter. I look forward to your prompt response.

I refer to my letter of Oct 17, 2023 02:10:22 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Request A copy of my Income Tax Return (ITR).

In reply, kindly clarify the following:

1. What kind of Income Tax Return are you requesting from us, Is it BIR Form No. 1701 or BIR Form No. 1701A?
2. When did you file your Income Tax Return?
3. Did you file it Online (Thru eBIRForms Facility) or Manually at the Revenue District Office (RDO) where you are registered?
4. What is your Taxpayer Identification Number (TIN)?

Email the requested information at sherry.ann.arcita@bir.gov.ph and indicate in the email subject ticket number #BIR-648879800897, to respond.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Jan 11, 2024 12:55:17 AM, your request will be closed.
